Gynecomastia surgery in Istanbul and Izmir, Turkey, liposuction with gland excision
Gynecomastia surgery flattens an enlarged male chest by removing the firm glandular tissue behind it and the fat around it — something diet, exercise and a tighter top cannot reach once true gland tissue has developed. JFS Clinic plans your surgery in Istanbul or Izmir, Turkey; your package includes one night in hospital and three in the hotel, from £2,550.

Am I a candidate?
Who is a good candidate for gynecomastia surgery?
Losing weight can shrink the fat layer across the chest, but it cannot shrink the firm glandular tissue that sits behind the nipple in true gynecomastia — that tissue responds to hormones, not to the gym. Gynecomastia surgery is the treatment that removes it directly.
True gynecomastia vs chest fat, why the gym alone rarely flattens it
An enlarged male chest usually comes from one of two things, often both together. Pseudogynecomastia is simply fat sitting over the chest muscle, the same way fat settles anywhere else on the body, and it can respond to weight loss like any other area. True gynecomastia is different — glandular breast tissue grows behind the nipple, triggered by hormonal shifts during puberty, by certain medications, by anabolic steroid or testosterone use, or sometimes by no clear cause at all. Once that gland tissue has formed, sit-ups and chest presses cannot shrink it, because it is not fat and does not respond to exercise the way the surrounding tissue does. Surgery is the only way to remove the gland itself.
Signs gynecomastia surgery may suit you
- A firm, sometimes tender lump or ridge directly behind the nipple that stays despite a stable, healthy weight.
- Chest size or shape that makes you avoid swimming, changing rooms or fitted T-shirts.
- Your weight has been stable for several months, so a surgeon can judge how much is gland and how much is fat.
- You don't smoke, or can stop well before and after surgery — smoking slows healing and raises the risk of complications.
- You are in good general health, with no untreated hormonal condition that could cause the gland tissue to return.
| Pseudogynecomastia (fat only) | True gynecomastia (gland tissue) | |
|---|---|---|
| What's enlarged | Fat over the chest muscle, like anywhere else on the body | Firm glandular tissue behind the nipple, plus often some fat |
| Feel on examination | Soft, even, no distinct lump | A firm, rubbery disc or ridge directly under the areola |
| Response to liposuction alone | Usually enough on its own | Not enough — the gland itself needs removing |
| Typical candidate | Stable weight, soft chest fat, no firm lump | Firm lump present, often since puberty or linked to medication |
What to discuss at your gynecomastia consultation
- Whether your enlargement is mostly fat, mostly gland, or a mix of both, judged by examining your chest directly.
- Whether liposuction alone, liposuction with gland excision, or skin excision fits your chest (see the comparison below).
- Any current or past medication, supplement or anabolic steroid use that could be driving gland growth.
- Whether chest liposuction should be combined with contouring elsewhere, such as the abdomen or flanks, in the same operation.
- Your full medical history, including any previous chest surgery or hormonal investigation.
This is general guidance, not a diagnosis. Your surgeon has the final say once they've seen your photos and your health history.
Not every gynecomastia case is the same
Types of gynecomastia surgery, liposuction, gland excision and skin removal
"Gynecomastia surgery" covers more than one operation. Your surgeon picks the technique based on how much of the enlargement is gland rather than fat, and how much the skin has stretched — not simply what you ask for.
- 1Small incision at the areola edge
- 2Gland tissue removed
- 3Surrounding fat liposuctioned
- 4Chest contoured flat
How gynecomastia surgery is done
| Technique | Best for | How it's done | Often combined with |
|---|---|---|---|
| Liposuction only | Pseudogynecomastia, where the enlargement is mostly fat | A thin cannula removes fat through small chest incisions, no gland touched | Abdomen or flank liposuction in the same visit |
| Liposuction with gland excisionMost common | Grade I–II (Rohrich), a firm lump of gland tissue plus some fat around it | Gland removed through a small incision at the areola edge, fat suctioned around it | Nothing further — usually a stand-alone procedure |
| Skin excision with gland removal | Grade III–IV (Rohrich), significant gland enlargement with stretched, sagging skin | Excess skin is removed along with the gland, sometimes with a free nipple graft | A longer, more involved single operation |
| Gynecomastia with body contouring | Patients wanting the chest and another area treated in one trip | The chest technique above, plus liposuction or a tummy tuck elsewhere | Abdominal liposuction or a tummy tuck |
How much is gland and how much is fat always depends on your chest on examination — your surgeon confirms the plan with you before surgery.
Liposuction-only vs gland excision, how your surgeon decides
The choice is not about which technique sounds simpler, it is about what your surgeon finds when examining your chest. Pinching and pressing behind the nipple shows whether there is a firm, rubbery disc of gland tissue or just soft, even fat — a firm lump means liposuction alone will leave the ridge in place, because a cannula cannot break up gland tissue the way it breaks up fat. Soft, even enlargement with no distinct lump often responds well to liposuction on its own.
Gynecomastia is sometimes caused by something ongoing rather than a one-off hormonal shift at puberty. Anabolic steroid or testosterone use, certain prescription medications, liver conditions and some recreational drugs can all stimulate gland growth, which is why your surgeon reviews your medication and supplement history carefully — removing the gland without addressing an ongoing cause can let it grow back.
How much skin excess is present is judged by how far it stretches when gently pulled, not just by chest size. A moderate lump with skin that springs back on its own usually needs only the small areola-edge incision used for gland excision; skin that stays loose and sagging after the gland and fat are removed is what moves the plan toward skin excision, sometimes with the nipple repositioned or grafted if the chest has been enlarged for a long time.

What can delay your gynecomastia surgery date
Being free to travel within a week doesn't mean every date is the right one for your body. A handful of factors decide your safest date, and your surgeon checks each one before the operation goes ahead. If your blood tests or the Day 2 consultation suggest waiting would be safer, the date moves — your safety always comes first.
| Factor | Why it matters | What usually happens |
|---|---|---|
| Smoking or nicotine | Nicotine narrows blood vessels and slows healing around the incision | You are asked to stop for several weeks before and after surgery |
| Blood thinners and some supplements | They can increase bleeding during and after surgery | Your surgeon tells you what to pause and when. Never stop prescribed medication without your own doctor's advice |
| Weight still changing | Fat regained later can sit over the treated area again | Surgery is planned once your weight has been stable for a while |
| Anabolic steroid or testosterone use | It can keep driving gland growth, raising the risk of regrowth | Your surgeon discusses stopping it and the timing before surgery |
| A recent chest injury or infection | Operating on recently affected tissue can change how it heals | Your surgeon checks the timing against any recent injury |
If you live in the UK, the NHS only funds gynecomastia surgery in limited cases where true gland tissue and real distress are both confirmed, so most men weigh up private clinics at home and overseas instead. Wherever you book, three questions are worth asking first: how quickly you can be seen, what happens if a test result pushes your date back, and who's available to help once you're back home.

What changes the price: needing skin excision for a larger, grade III–IV case, adding a free nipple graft, or combining gynecomastia with liposuction elsewhere on the body. A smaller, liposuction-only case can sometimes cost less than the standard package; skin excision, a combined operation, or surgery for a long-standing, firm gland typically costs more because of the extra surgical time. Your final price is set once your surgeon has reviewed your photos and your Day 2 consultation notes.
The procedure
How gynecomastia surgery is done, step by step
Gynecomastia surgery with gland excision usually takes around 1.5 hours. Here are the steps in order, from anaesthesia to the compression vest, followed by 1 night of hospital nursing care.
Your surgery usually takes place under general anaesthesia, or local anaesthesia with sedation for a smaller, liposuction-only case. Before you go under, your surgeon marks your chest while you are standing — tracing the edge of the areola where the gland-removal incision will sit, and the entry points for the cannula, based on the plan agreed on Day 2.
If liposuction is part of the plan, it is usually done first, working through small entry points around the chest to soften and remove the fat layer surrounding the gland, which makes the gland itself easier to see and reach once the surgeon moves to the next step.
Inside the procedure

- 1Anaesthesia and markingsChest marked and anaesthesia started while you stand.
- 2Surrounding fat liposuctionedA cannula loosens and removes fat around the gland.
- 3Gland tissue excisedGland removed through a small areola-edge incision.
- 4Incision closed, vest fittedTiny stitches close the cut; compression vest fitted.
Gynecomastia gland removal
Once the fat has been thinned around it, the gland itself is reached through a small incision placed right at the lower edge of the areola, chosen because the natural change in skin colour there hides the resulting scar well. Working through this incision, your surgeon identifies the firm, disc-shaped gland tissue and excises it directly, taking care to leave a thin, even layer of tissue behind the nipple rather than removing everything flat against the muscle, which helps avoid a sunken or dented look once healed.
Once enough gland and fat have been removed to leave a flat, natural-looking contour, the small incision is closed in layers with fine sutures, placed to sit discreetly at the areola edge. A compression vest goes on in theatre, straight over the chest, to help the skin settle against its new contour and control swelling from the first moment. Drains are only used for larger cases or when skin excision is also needed; a straightforward liposuction-with-excision case usually does not need one.
Throughout the procedure an anaesthetist watches your heart rate, oxygen levels and blood pressure, adjusting your anaesthesia as the surgery moves between liposuction and gland excision. A single-sided or straightforward case typically takes around an hour to an hour and a half; treating both sides with a larger gland, or adding skin excision, takes longer simply because there is more surgical ground to cover.
Once your vest and dressings are on, you are moved to a recovery area where nursing staff watch you wake from the anaesthetic before you go to your hospital room for your one night under observation. Most patients are up and walking slowly with help later the same evening or the next morning, which helps circulation and reduces swelling from the very start.
Recovery, day by day
Gynecomastia recovery time, week by week
Move the slider from Day 1 to month 6. For each stage you see how most men feel, what is realistic to do (around 2 weeks to a desk job) and what to avoid, such as chest training before your surgeon clears it.
Recovery from gynecomastia surgery happens faster than from surgery that removes large volumes of skin, since the incision is small and there is no muscle repair involved. The first week is about letting swelling and bruising settle while the compression vest holds the new, flatter shape in place. Weeks two to six are about the chest looking progressively more even as swelling drops; upper-body training usually returns gradually from around week six, once your surgeon clears it. From six weeks onward, the focus shifts from healing to the chest settling into its final, flat contour.
The vest is not just comfort — it controls swelling and helps the skin settle evenly against the new contour, which is why it stays on almost all day for the first week or two and only moves to part-time wear once your surgeon is confident the swelling has started to even out. Some numbness or heightened sensitivity around the nipple is common early on and usually settles within a few months as the nerves recover.
Typical timeline for liposuction with gland excision. Your surgeon may give you a different plan, especially if skin excision or combined body contouring is involved.
| Activity | Week 1 | Week 2 | Weeks 3–4 | Week 6 | Months 3–6 |
|---|---|---|---|---|---|
| Walking | Short, slow walks with help | Walking further each day on your own | Longer walks at a normal pace | No restriction | No restriction |
| Desk work | Possible from day 5–7, vest on | Most men are fully back at a desk | Full days comfortable | No restriction | No restriction |
| Driving | Avoid — reaction times are affected by pain relief | Confirmed by your surgeon at your check-up | Set by your surgeon after your check-up | No restriction | No restriction |
| Flying | From day 4–5, after your check-up, wearing your vest | Confirmed by your surgeon at your check-up | Set by your surgeon after your check-up | No restriction | No restriction |
| Gym, chest exercise | Not yet | Avoid — no swimming or lifting yet | Still avoid — no chest or shoulder work | Gradual return, starting light | Normal exercise, as cleared by your surgeon |
| Vest | Worn almost all day and night | Worn most of the day | Worn most of the day | Coming off, or worn only part-time | As advised by your surgeon |

In their own words
Ellie's helped me all the way through with my surgery. Every time I've messaged her, she's got either her or my assistant as well to come and bring me whatever I've wanted whenever I wanted. So I'm very happy.
What to expect
Gynecomastia results and when they settle
Most of the early swelling is gone within about six weeks, though the chest keeps softening and settling into its final shape for up to three to six months as deeper swelling resolves. Many men notice a flatter, more defined chest within the first month, while the smoothest, most even result — especially close to the nipple — takes longer to show fully.
The small incision at the areola edge fades quickly and is usually barely visible once healed, hidden by the natural change in skin colour there. Some mild unevenness or firmness around the nipple is common in the first few months as the area settles, along with occasional changes in nipple sensitivity that usually recover over time; gentle massage, if your surgeon recommends it, and patience with the swelling timeline both help the final contour show through.
Results vary from patient to patient. Your surgeon will tell you what is realistic for your body at the consultation.

Questions
Gynecomastia surgery in Turkey, frequently asked questions
How much does gynecomastia surgery cost in Turkey?
At JFS Clinic, the gynecomastia package is listed from £2,550 (€2,950 / $3,400) and includes surgery, one night in hospital, three nights in the hotel and all transfers. Larger cases needing skin excision, or gynecomastia combined with liposuction elsewhere on the body, move to a personal quote once your surgeon has reviewed your photos.
See all package prices →How soon can I have gynecomastia surgery in Turkey?
Most JFS patients are in Istanbul or Izmir for gynecomastia surgery within about a week of their first message. You send two photos of your chest, front and side; your coordinator comes back with a personal plan, the price and the dates your surgeon has free. The exact date is only confirmed once your blood tests and the Day 2 consultation are done, and it moves if your surgeon decides waiting would be safer.
From first message to surgery →Can I get gynecomastia surgery on the NHS?
Sometimes, if a doctor confirms the enlargement is true glandular gynecomastia rather than fat alone, and it is causing marked physical or psychological distress. Funding decisions and waiting times vary a lot by local integrated care board, which is why many men also compare private options, in the UK and abroad, while they wait for an assessment.
Is it safe to go to Turkey for gynecomastia surgery?
Safety depends mainly on your surgeon and the hospital, not simply on the country you pick. Op. Dr. Onur Evren Yılmaz is a specialist plastic surgeon working in a hospital licensed for international patients, and JFS Clinic itself holds a health-tourism licence. Blood tests and a pre-operative check on Day 2 confirm you are fit for anaesthesia before anything goes ahead. Before you book, ask about your surgeon's training, the hospital's licence and how aftercare is arranged.
Which country is best for gynecomastia surgery?
No country can honestly claim to be the best for every patient, so compare clinics on standard points: the surgeon's specialist training and experience with male chest contouring, a hospital licensed for international patients, a written price stating what it includes, and follow-up once you are home. At JFS Clinic you see your surgeon's name before booking and meet them before surgery. Op. Dr. Onur Evren Yılmaz, a plastic surgeon who completed his specialist training in 2012, operates, and your coordinator passes questions and photos from home to him on WhatsApp.
Why is Turkish surgery so cheap?
Price gaps come mainly from lower hospital and staff running costs and an exchange rate that favours pounds, euros and dollars, not from a different operation. A lower price is no proof of a weaker result, and a higher one is no proof of a better result, so check the same standard points: the surgeon's training, the hospital licence, what the quote includes and the aftercare plan. JFS lists its gynecomastia package with surgery, hospital nights, hotel and transfers, and names your surgeon before you book. Flights are not included.
On a scale of 1 to 10, how painful is gynecomastia surgery?
You are asleep or sedated for the operation itself. Afterwards, most men describe the chest as tight, bruised and tender to the touch, similar to a hard chest workout followed by deep muscle ache, easing over the first couple of weeks as swelling goes down. Pain relief is provided throughout, and how sore it feels still varies from person to person.
What's the difference between liposuction-only and gland excision for gynecomastia?
Liposuction alone removes fatty tissue through a thin cannula and suits pseudogynecomastia, where the enlargement is mostly fat rather than gland. True gynecomastia also involves firm, fibrous glandular tissue that liposuction cannot reach on its own, so it is removed through a small incision at the edge of the areola, usually combined with liposuction around it for a smooth result. Your surgeon decides which applies to you by examining your chest at the consultation.
Can gynecomastia come back after surgery?
The glandular tissue removed during surgery does not grow back, so a true recurrence is uncommon. Significant weight gain afterwards can add fat to the chest again, and ongoing causes such as anabolic steroid use, testosterone therapy or certain medications can trigger new glandular growth even after a successful operation, which is why your surgeon reviews your medication history carefully beforehand.
How long after gyno surgery can I fly?
After your surgeon's final check-up, usually on day 4 or 5 of your trip. Wear your compression vest, walk up and down the aisle during the flight, and avoid lifting your own luggage.
How long after gyno surgery can I fly?
Your surgeon decides when you are fit to travel, rather than a fixed number of days. In the JFS plan you spend five days in Turkey, and on Day 5 you have a dressing and healing check at the clinic before you are cleared to fly home. You then receive your aftercare kit and flight advice: an aisle seat, regular short walks and your compression vest on.
How soon will I see my final gynecomastia result?
Initial swelling takes a few weeks to settle, and most men see their flatter chest contour taking shape by around six to eight weeks. The chest continues to soften and settle for up to six months as deeper swelling resolves. Results vary from patient to patient. Your surgeon will tell you what is realistic for your body at the consultation.
